Our Primary Care Clinics are open seven days a week, from 8:30 am to 4:30 pm week days and 9 am to 4 pm weekends, to provide family medicine, urgent care and occupational medicine.

Appointments
Appointments for routine care (annual physical examination, well child care, etc.) are made by the appointment 972-262-1682.

Please call at least four weeks in advance for routine visits so that we can schedule these with your provider. Because many of our early morning calls are for illness care, please delay calls for routine care until late morning or afternoon.

Same Day Appointments
Appointments for same day treatment of medical problems (illness/injury care) are made through our triage nurse from 8:30 a.m. to 12:30 p.m. In the afternoons, calls for urgent problems are assessed by the team nurse. We try to ensure continuity of care with your doctor. If your doctor is not available, it may be necessary to schedule urgent visits with another provider on your team.

First Time Patients
First time patients should arrive 15 minutes early for their scheduled appointment to allow for registration. To save time, fill out our new patient registration form and bring it with you.

Before your first appointment, call your doctor's office and have your records forwarded to our office. You will also need to contact your insurance company to change your primary care provider.

Call Ahead of Service
It's like being here before you get here. Call us right before you're coming. We'll put you on the list. When you get here, just let us know you've arrived.

We also have a Call Waiting Service. If you want to leave the center before you've been seen, give us a phone number to contact you. We'll call you to return when we're ready to see you.

Cancellations
Call the office as soon as possible if you are unable to keep an appointment. Failure to cancel an appointment prevents other patients from being seen and increases the cost of care. If you are more than 15 minutes late, your provider may have to ask you to reschedule your appointment.

For Emergencies Office Hours are:
Mon - Fri 8:30 a.m. - 5:00 p.m.

One of our doctors is always available for emergencies. If you have an emergency after regular office hours, please call 972-262-1682 and a taped recording will give you the number of our answering service. The answering service personnel will ask for information about your medical problem and will take your phone number. The doctor on call will contact you within one hour, sooner for serious emergencies

Dedicated Professional Staff
Most Reliant Health physicians are board-certified in Family practice and Internal Medicine. If you need further evaluation, or specialized treatment, we will refer you to a specialist. We have an excellent relationship with many exceptional specialists in the area.

Complete Records
Starting with your first visit, we maintain a file of your medical records. Even if you visit a different Reliant Health location, your records are available through a system-wide computer network.During your each visit you will have access to a copy our HIPAA privacy Policy.

Reasonable Fees, Convenient Payment
Reliant Health services are priced competitively compared to other physicians' offices, and less expensive than hospital emergency room charges. We accept most health plans , VISA, MasterCard, American Express, cash or check. And you'll always get an itemized receipt.

Occupational Medicine Services
Reliant Health offers injury treatment as well as a wide range of cost-effective medical exams, screenings, and certified MRO services. We customize them to all types of businesses and industries, and to each employee's medical and work history. Our mission is to cut the cost of job-related injuries. With so many locations, it's possible for employees to get the attention they need with minimal time spent away from the work site.

Prescription Refills
If you need a refill on your medications before your next appointment, please give us at least one week advance notice. Call between 10:00 a.m. and 11:50 a.m. and ask for your team secretary. We will need the:

  1. Name of the medication
  2. Dosage (mg.)
  3. Pharmacy name
  4. Pharmacy phone number
  5. Pharmacy address
  6. Prescription number
  7. Directions for use

The secretary will take your message, get your chart, and give it to your provider. He/she will review the medication request within one a week, and will either have the prescription ready for you to pick up at the center or will call the pharmacy with the renewal.

Emergencies and Hospitalization
If you are experiencing a life-threatening emergency, call 911 or go to the nearest emergency room and have the hospital notify our office.

Most HMO insurance plans require prior approval of all emergency room visits. Please call our office before obtaining these services.
